I will try to answer all of your questions here. I ordered my phone from HKS (VERY highly recommended, among the best online purchases I have ever made) and it came in the funky rectangle box with all NFS, AoE II, etc. and 2 stylus along with the rest of the normal software and wall charger. A quick note about the charger I received. It appeared to have 3 huge prongs that obviously would not fit the sockets here in the US. They also sent an adapter, but it was the wrong adapter for the US as well. Upon further inspection, there was a small screw below and right of the prongs. When removed, the piece with the prongs slides up off the top and VOILA! there's the US 2 blades that you flip down to plug in the wall. Anyway, I am on Cellular one and I had no problems setting the phone up and it works flawlessly. It has the best reception of any phone I have ever owned. I was also concerned about scratches and such on the screen, so I ordered a screen protector from:

If you search you can find great reviews on them. I also ordered a clear plastic case for it which turned out to be a great bargain. It came with the plastic case, a screen protector that seems identical to the brando one, and a small microfiber cloth for wiping the screen....all for $4.99 shipped. Look on Ebay for seller:

For some reason you cannot access the 4GB cards through the phone in mass storage mode. This seems to be a common issue with all 4GB cards. All you have to do is stick the card in the phone first and let the phone format the card. Then you have to use an SD card reader on your computer to read/write to the card and it works like a champ! Also make sure that any time you are taking the card out of the phone you "eject" it through the system menu....that will make more sense once you have the phone and read the manual.
